Retirement
This is to inform you that I will be retiring in December 2025. I wanted to provide you with some resources for locating a therapist in the future if needed. Records are kept for 7 years from the time of your last appointment. Any records requested by a new therapist must be accompanied by the release of information consent form included on this website under Patient Forms.
